To
determine if your health insurance plan provides coverage and payment
for LUXÍQ® Foam, simply call the toll-free number on your insurance card,
provide your plan name and number, subscriber number, and your diagnosis.
Provide the name and the NDC # of the product.
In order to receive prior authorization
for LUXÍQ® Foam, your prescribing physician needs to submit prior authorization
information to your insurance company. Most prior authorizations are
approved or denied within 24 hours to 72 hours. If you cannot obtain

Your prescription drug plan may have
a closed formulary, which means that only certain drugs are covered.
If LUXÍQ® Foam is not listed as a covered drug, the prescribing physician
may be able to request an exception for coverage for LUXÍQ® Foam. These
are awarded if a physician can document that there are no other drugs
available on the formulary to treat the condition, or that a drug
listed on the formulary was tried but the drug failed to treat the
condition effectively. If you cannot obtain an exception for coverage

Ask your physician to indicate "Dispense as Written" on the prescription
and make sure to tell your pharmacist you want LUXÍQ® Foam as there
is no generic equivalent for LUXÍQ® Foam.
